The role of the district nurse has changed over the years, and we are now looking after patients in their home environment including complex wound management, palliative care and end of life care, medicines management, as well as having a high standard of holistic assessment and care planning skills.
You will be working independently to support with managing the care of patients in our local community and wider Neighbourhood.

Main duties of the job
This post will involve working between the hours of 08:00 -- 22:00 including weekends and bank holidays on a rota basis.
Shift patterns include: 8am-4pm, 9am-5pm, 10am-6pm, 2pm-10pm. We are open to hearing from applicants who are looking for either full or part-time hours on a permanent basis.

* Work closely with the Team Leader coordinating the community nursing teams off duties to meet the needs of the community nursing service/workload.
* Ensuring all loans of equipment are appropriate and reviewed regularly.
* Working in partnership with other health care professionals and agencies.
* Encouraging client independence and empowerment, recognising and respecting their involvement in the planning and delivery of their care.
Person Specification
Qualifications
* Registered General Nurse
* Level One Registration with appropriate registration with the NMC as per post requirements
* Teaching/Assessing Qualification
Experience
* Required to develop the knowledge base in line with the KSF requirements for the post.
* Evidence of Continuing Professional Development and to have undertaken post registration training.
* Experience of working in multi-agency services.
