The shift pattern for this position is 5 days a week.
An exciting opportunity has arisen for a community staff nurse to join the Diabetes Specialist Nursing service in East Dunbartonshire HSCP. The successful candidate will join this dynamic team and support the Diabetes Specialist nurses to manage a complex caseload of people with diabetes.
They will contribute to the delivery of diabetes education and support adults, their families, carers and wider MDT Teams across the primary and secondary care interface.
They will be responsible for the assessment, planning, delivery and evaluation of high quality nursing care programs which includes nurse led home visits, clinic appointments and educational sessions. Candidates must be on the NMC register as an Adult nurse with a minimum of 2 years post registration experience and can demonstrate an interest in diabetes management.
