Adult Social Work Services are recruiting for a Team Manager who will be an experienced social work practitioner and part of a new fully integrated multidisciplinary leadership team based at East Lothian Community Hospital. This team comprises social workers, community care workers, physiotherapists, occupational therapists, care at home, and the third sector. The role focuses on a coordinated pathway approach to preventing hospital admissions and facilitating timely discharges, thus improving therapeutic outcomes by delivering rehabilitative care at home.
You will oversee the main intake social work system in community adult social work services based at John Muir House in Haddington, ensuring harmonisation between both systems for seamless progression of individuals through this pathway into locality social work services.
East Lothian Health and Social Care Partnership are adopting approaches that align with the new National Frailty standards and coordination of care via locality-based approaches with close links to the third sector.
This position will involve some weekend work, requiring you to work across 7 days initially over a 5-day working week, subject to review once the new pathway has been embedded. This post is onsite, with limited scope for home working.
Interviews will take place on February 20th.
