Job summary We are looking to welcome a caring professional - ANP, ACP, Paramedic or Pharmacist with Minor Illness skills into our practice, to work full/part-time. The main responsibilities of the role include delivering a high standard of patient care, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base. Assessing, diagnosing and treating patients conditions, initiating direct referrals as appropriate. Main duties of the job Ideally you will be an independent prescriber but this is not essential for the right candidate. You will be able to telephone triage, manage e-consults, working as part of an acute hub, as required. As a vital part of our multidisciplinary team you will be able to support and care for a diverse patient group, across our four site practice. They are supported by an invaluable admin and reception team. Our main surgery is at St Bartholomew's, on Manzil way. To maintain accurate contemporaneous computerised clinical records of every contact, including history, examination, diagnosis, plan of care, advice given, treatment and recommended follow up. This includes documenting any adverse event, safeguarding child or vulnerable adult issue or other notifiable matter. Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition. To be flexible and responsive both to the clinical needs of patients and operational needs of the service. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate. Undertakes annual review of patients with chronic disease in the community as identified. Undertake medication reviews in line with practice protocol as identified, face to face with patient.
