Practice Nurse
We are looking for an enthusiastic, committed and experienced practicenurse or nurse practitioner to join our clinical team and work with us in a friendly, well-established practice. The practice is flexible with working hours and bothfull time and part time applicants are welcome to apply.
Previous NHS generalpractice nursing experience is essential. Good working knowledge and proficiency with all Microsoft Office applications essential.EMIS Web knowledge desirable.
Hours
The successful applicantwill need towork between 10 to 36 hours per week, Monday to Friday.
Salaryand benefits
£42,000 - £52,000 full time equivalent, depending upon experience

Main duties of the job
You will work as part of the practices clinicalteam and be responsible for delivering a high standard of nursing care topatients. You will have excellent clinical skills, ideally with experience across mostaspects of nursing in general practice or a willingness to expand your knowledge and skills, including:
- Chronic disease management (LTCs)
- Cervical screening and womens health
- Childhood immunisations
- Travel immunisations
- Wound care
- Phlebotomy
- Spirometry
- Ear irrigation
- Family planning
- New patient health checks and NHS health checks

The practice's aim is to provide ourpatients with an excellent standard of patient-centred care, with cliniciansand patients jointly involved in all aspects of planning and delivering care tomeet patients' health and well-being needs.

Job responsibilities
Clinical responsibilities:
Implement health promotion and wellbeing programmes
Develop and implement individual care plans for chronic disease patients
Manage and support patients at risk of developing long-term conditions, providingadvice to promote the patients health
Provide wound care
Run specialist clinics such as ear syringing, contraception etc.
Collect specimens for pathology
Provide travel advice and vaccinations
Request pathology as necessary
Process and interpret limited test results as required ( cervicalcytology results)
Provide LTC clinics and refer patients to secondary care as required
Maintain accurate contemporaneous clinical records in line with current regulations
Ensure data is coded appropriately
Implement an effective call/recall system
Implement an effective immunisation programme in line with the nationalschedule
Chaperone patients as necessary
Support the clinical team in dealing with clinical emergencies
Contribute to practice targets (QOF, local targets, etc.), complying withguidance as necessary
Liaise with external services and agencies to ensure vulnerable patientsare supported (safeguarding)
Supplementary responsibilities:
Provide nursing expertise and guidance as necessary
Act as the infection prevention and control lead for the practice,ensuring compliance with all relevant legislation and regulations, undertaking periodicaudits as necessary
Monitor and ensure the safe storage, stock rotation and disposal of emergencydrugs and vaccines
Support and participate in shared learning within the organisation
Support and participate in the practices significant event reviews and processes

Experience
Essential
1. Experience of working in general practice as a practice nurse
2. Experience of working autonomously
3. Comprehensive understanding of infection prevention and control measures
Desirable
4. Experience across the range of primary care nursing duties, including long term conditions
5. Experience of prescribing and undertaking medication reviews
6. Experience of working in a supervisory or mentoring role
Other requirements
Essential
7. Disclosure Barring Service (DBS) check
8. Occupational health clearance
9. Met the requirements for and completed revalidation with the NMC
10. Evidence of continuing professional development (CPD) commensurate with positions held
Qualifications
Essential
11. Degree in nursing
12. Registered nurse on the nursing and midwifery council register
Desirable
13. Qualified independent nurse prescriber
14. Postgraduate diplomas or degrees (in clinical areas such as chronic disease management)
15. Registered for other relevant roles (cervical screening, spirometry)
