About Anglia Case Management

We have a wonderful opportunity for an experienced Rehabilitation Case Manager in Norwich and surrounding areas to join our friendly team. We work with adults and children with ongoing needs to support them in improving their quality of life and maximizing their potential. We are committed to creating individual solutions for people affected by injury or illness, providing expertise to help clients adapt to their circumstances.
Qualifications and Requirements
* Must be a driver with the ability to travel
* Hold a UK professional health qualification (HCPC, NMC, or Social Worker Registered)
* Experience or membership in the British Association of Brain Injury Case Managers (BABICM) is advantageous
Our Services
Our services include Case Management, Expert Witness reports, Rehabilitation, Manual Handling assessments and training, and Mental Capacity Act assessments. We work with clients with disabilities such as brain injuries, spinal injuries, cerebral palsy, amputation, complex orthopaedic injuries, stroke, dementia, and PTSD.
Role Details
Working Hours: Flexible hours, Monday to Friday, with total hours per week to be discussed.
Key Responsibilities
* Advocating for clients and building relationships with families, deputies, and solicitors
* Producing action plans and goals with specific timeframes
* Recruiting, selecting, and managing care/support workers
* Implementing and reviewing Support & Rehabilitation Plans
* Ensuring flexible support team working patterns, including out of hours
* Conducting risk assessments and managing safety strategies
Candidate Requirements
* UK professional health qualification (HCPC, NMC, or Social Worker Registered)
* Full driving license and access to a car
* Experience supporting adults or children as a Case Manager
* Knowledge of CQC standards, MCA, DoLS, and safeguarding frameworks
* Strong organizational and planning skills
* Flexible, professional, and positive attitude towards multi-site working
