You will work alongside GPs with specialist interest in Dermatology and a well-established, experienced nurse-led care delivery team.
Working with the Community Dermatology Team, you will play a key role in driving improvements and innovations in the management of patients with a range of skin conditions.
You will need to engage with all members of the multi-disciplinary team to advise, support and educate, developing your inter-personal and leadership skills.
The successful applicant will have a passion for improving patient care, enjoy project work and understand the importance of good communication and evidence-based practice to improve patient safety, patient experience, and clinical effectiveness.
Main duties of the job
Applicants will be an expert practitioner, providing clinical support and advice to healthcare professionals where necessary to assist them in the management of dermatology conditions.
You will also be responsible for the assessment, planning, implementation, and evaluation of specialist programmes of care.
Provide direct, skilled, and individualised care for patients and their carers, including clinic consultations and minor surgery intervention.
Act as a role model and provide support, governance, and ongoing professional development within the team.
Job responsibilities
Clinical Nurse Practitioner - Part Time 30 Hours Per Week
* To coordinate the delivery and development of the specialist service through the use of effective evidence-based practice in line with agreed national standards for patient safety and monitoring.
* To contribute to the organisation of multidisciplinary, patient-centred care.
* To be accountable for the planning and delivery of safe, effective and compassionate patient care in relation to dedicated pathways of care, working as an autonomous practitioner within the specialism.
* To provide specialist clinical advice and support to patients and clinical staff within the Trust on the assessment, management and care of patients within the specialist area.
* To continuously improve clinical care standards in line with the clinical governance framework.
* To exercise accountability as set out in the NMC/HCPC Code.
* To ensure effective communication internally and externally regarding patient care and resources.
* Provide education and training to clinical staff across the Trust.
* To provide direct access for patients, either face to face or telephone consultation.
* To represent the service nationwide.
* To play a key role in ensuring that national and local standards/targets for specialist area are met.
* To manage/lead practitioner-led clinics in relation to reviewing patients and play a key role in optimising complex treatment regimes, facilitating hospital discharge and preventing avoidable hospital admissions.
* To undertake research relevant to the needs of the specialist area.
Person Specification
Skills and Knowledge
* Awareness of national health and social care policy.
* Knowledge of best practice in area of specialism.
* Knowledge of current professional and clinical governance issues.
* Thorough understanding of expanded roles / professional practice issues.
* Clinical teaching experience.
* Ability to provide and receive complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or reassurance skills are required.
* Able to work without supervision.
* Competent in the use of IT packages.
* Evidence of competency in relation to clinical practices appropriate to the post.
Qualifications
* Registered Nurse (adult) or Allied Health Professional.
* Degree in nursing studies or healthcare related subject (UK equivalent).
* Mentorship certificate / Practice Assessor training or demonstrable equivalent experience.
* Non-medical prescribing or willingness to work towards.
* Leadership and Management course.
* Qualification in specialty.
* Level 7 surgical skills in dermatology.
Experience
* Extensive post-registration experience, some of which will be at a senior level within the required specialist area of practice.
* Experience of care delivery in nurse-led dermatology clinics.
* Experience of working in the specialism.
* Experience of leading a team/supporting junior staff.
* Evidence of innovation in practice.
* Evidence of change management.
* Evidence of practice development.
* Experience of coordinating care for a caseload of patients.
Physical skills
* Ability to assist patients to move as per Trust Moving and Handling Policy.
* Ability to meet the travel requirements of the role.
