Clinical Nurse Specialist - Inflammatory Bowel Disease

An opportunity has arisen for an experienced Paediatric Nurse to join our Gastroenterology nursing team.
The role involves being part of a large MDT, providing specialist care to children and young people with IBD. The successful candidate must be:

The post will include both patient facing and office-based work. The successful candidate must be familiar with Microsoft Word and Excel to co-ordinate patient appointments and maintain databases.
The role will include:
* Providing disease and medication information to newly diagnosed patients and families
* Co-ordination of inpatient and outpatient care
* Provide specialist education to other health care professionals
* Liaising with schools, GP's and community nursing teams as required
* Reviewing and co-ordinating ongoing care for patients attending Medical Day Care
* Nurse led clinics
* Supporting children, young people and parents who have been diagnosed with a chronic condition
* Healthcare Transition - helping to prepare young people for adult IBD services through face-to-face transition discussions, live transition plans, nurse led clinics and transition clinics.
Main duties of the job
* To act as a Named Nurse and plan, implement and evaluate care for children with confirmed gastroenterological and hepatology conditions
* To develop and co-ordinate the Gastroenterology nursing Service in a variety of settings ensuring the quality of care provision for Children of all ages.
* To provide specialist education, training and information to children, families and members of the multi-disciplinary team.
* Provide a seamless transition of care for patients with gastroenterology and liver conditions to adult services.

Qualifications and Training
* RSCN or RN (Child)
* Paediatric Life Support Certificate
Experience
* Experience of caring for children and families with gastroenterology conditions
* Medical and/or Surgical Nursing experience
Knowledge and Skills
* Knowledge of Holistic Family centred care using evidence based practices
* Health & Safety Safe Practices e.g. Lifting and Handling, Sharps, COSHH
* Current Nursing/Health Issues
* Infection control
* Management of own Client Group
* Interest or experience in audit and research
Personal Attributes
* Ability to work alone and as part of a team
* Flexible and adaptable
* Awareness of own learning needs - proactive approach to learning
* Willingness to develop skills
