What you will do:
1. Plan, implement, review, and optimise evidence-based interventions tailored to individual care needs and associated risk management.
2. Deliver direct care and assistance to individuals facing mental health challenges in a community context.
3. Take accountability for a caseload of complex cases, ensuring thorough management and ongoing support for service users.
4. Engage actively in case conferences and multidisciplinary team meetings, promoting involvement from service users and their carers throughout the care planning process.
5. Advocate for a person-centred approach, upholding service users' rights and choices while ensuring their voices are heard.
6. Assess risks efficiently and develop effective risk management plans to ensure safety.
7. Provide duty cover and offer support to service users via telephone or face-to-face interactions.
8. Mentor and guide junior staff and student nurses, supporting their career growth and professional development.
9. Offer clinical advice to peers within the CMHT to maintain high-quality care standards.
10. Actively contribute to enhancing service quality through service development initiatives.
Requirements:
1. Registered Mental Health Nurse (RMN) with valid NMC registration.
2. Extensive post-registration experience in a community mental health setting.
3. Proficiency in assessment, risk management, and care planning.
4. Demonstrated capability to manage a caseload of service users with complex mental health needs.
5. Strong ability to operate independently and as an integral part of a multidisciplinary team.
6. Excellent communication and interpersonal skills.
7. Comprehensive knowledge of relevant legislation and guidelines, notably the Mental Health Act and safeguarding protocols.
8. Experience in providing clinical supervision or mentoring to junior staff.
9. Additional specialist training pertinent to community mental health nursing.
10. Full UK driving licence and access to a vehicle for professional duties.
