Are you a caring and confident individual with enthusiasm? Do you wish to deliver excellent patient centered care?
We are looking for support workers, with previous hospital/community healthcare setting experience, to assist registered nurses and qualified therapists in delivering an excellent admission avoidance service as part of our Urgent Community Response Team (UCRT). This involves providing clinical interventions and supporting the recovery of patients who may be experiencing a health and social care crisis within their own home. The UCRT is a community admission avoidance team providing a 7-day a week service to support patients to remain within their own homes where clinically safe to do so. The team provides a 2-hour response to referrals and offers specialist assessments and interventions as well as planning ongoing care for both health and social demand. Support workers will be expected:
* To contribute to the assessment and treatment of individual patients on the caseload and communicate any changes in health and care needs to the registered staff.
* To communicate effectively with patients, relatives, carers, and health care professionals to assist the registered staff in ensuring an integrated, holistic approach to patient care.
The post holder will work as part of an integrated multi-disciplinary team.
The post holder will require a valid driving license and access to a car as the service covers patients living in the Mid Essex area.

* To gain informed consent and deliver sensitive, high-quality patient care
* To encourage patients and carers in an active approach to achieving their goals and maintaining independence
* To assist in identifying equipment and adaptations required in a patient’s own home
* To monitor patient progress and report back to the registered staff.
* To complete clinical records ensuring they are accurate, timely, and in accordance with the organisational, professional, and departmental standards and procedures.
* Maintain and develop knowledge, skills, and competence to ensure patients' individual needs are met in accordance with the band 3 competency framework.
