Job summary
A great opportunity exists for an experienced or newly qualified nurse who has a passion to deliver the best care for our patients on our wards. Patient safety is paramount therefore knowledge of the Nursing Process, healthcare governance, health and safety and infections prevention is required. We are looking for a motivated and enthusiastic individual with a passion for rehabilitation and excellent outcomes.
Snowdon is a 14 bedded unit on the Western Community Hospital site, which specialises in the treatment of adults with physical and cognitive impairment following a recent neurological event or a long- term neurological condition.
It is adjacent to the Kite Unit which is a 10 bedded purpose-built unit offering a neuropsychiatric service providing rehabilitation to individuals with a brain injury whose impairments are largely in the cognitive, behavioural, or mental health spectrum. The wards work very closely together to provide seamless care to patients.
As part of Solent's exciting Futures project, our physical rehabilitation wards Lower Brambles and Fanshawe will move across from the RSH Hospital bringing all our rehabilitation wards onto a single site in September 2024 at the Western Community Hospital with a new purpose-built rehabilitation centre of excellence - South of England Rehabilitation Centre.
Snowdon supports degree, apprentice nursing, AHP and medical students in practice.
Main duties of the job
*Assess patients /clients, plan develop/implement/evaluate programmes of care and carry out nursing procedures.
*Ensures use of nursing knowledge and skills to assess, plan, implement and evaluate patients/clients/family/ health needs as well as actively promote health and wellbeing.
*Evaluate clinical outcomes and reports variance to clinical pathway, protocols, or best practice guidelines.
*Contributes to effective discharge planning and/or referral processes to health care providers and support agencies to meet identified health needs.
*Applies diagnostic reasoning and professional judgement to nursing practice issues/decisions.
*Fosters inquiry and critical thinking amongst colleagues to advance nursing practice and patient/client care to ensure positive patient/client outcomes and person-centred care.
*Accurately documents assessments of patients/client's health status, diagnosis and response to nursing and other interventions and treatments on the Trust Clinical Records System.
*Effectively manage clinical care and clinical area through the implementation of clearly defined policies and procedures
*Educate and provides information to patients/clients/family to improve knowledge of disease/illness, self-management prevention of complications and promotion of recovery.
*Identifies when a vulnerable adult may be at risk and knows how to refer to the local authority adult safeguarding service.
*Role models culturally safe nursing practice, reporting poor practice when observed.
About us
We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.
Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.
The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.
Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.
