The Universal Crisis Team for Sunderland and South Tyneside Crisis Team are looking to recruit motivated and committed individuals into the Working Age Pathway.
The Working Age Pathway has 1.0 Full Time Band 5 Vacancy - based at Hopewood Park.
The post holder will be part of a team working under the crisis model and pathway, offering a 7 day a week service as an alternative to hospital admission. The successful candidate must have relevant clinical experience and ideally have a track record of clinical experience in working with risk and complexity.
The role requires sound clinical skills in all aspects of Home Based Treatment, including management of risk and therapeutic interventions.

Main Duties of the Job
The successful candidate will deliver care and treatment to patients that have been taken on for Home Based Treatment. This will involve visiting patients in the community who are in mental health crisis, offering various therapeutic interventions with ongoing risk management. Also engaging in daily MDT reviews, liaising with other professionals, and working together in a closely knit team.

Person Specification
Knowledge and Experience
* Knowledge of relevant contemporary professional theories of mental health care and evidence-based interventions.
* Understanding of the statutory issues which impact on service users.
Skills and Competencies
* Ability to use IT systems.
* Comprehensive continuous assessment of skills.
* Risk Management and Safety Planning skills.
* Ability to establish rapport and demonstrate engagement skills with service users.
Education and Qualifications
* 12 months post-registration experience and completed preceptorship.
* Experience of working with service users and carers in successful delivery of treatment plans.
* Completion of preceptorship programme for newly qualified nurses.
* Multi-disciplinary teamwork.
* Proven ability to develop therapeutic partnerships with service users and carers.
* Proven ability to communicate interventions to service users.
