This locum post has been created to expand our community palliative medicine Consultantprovision in East Berkshire. The role offers an excellent opportunity toinfluence palliative and end of life care services, and to make a meaningful difference to peopleat the end of their life and their families. This post is based at ThamesHospice delivering Specialist Palliative Medicine across East Berkshire.The post will provide Consultant leadership to the 5 bed virtual ward alongsideprovision of specialist medical review for complex patients on the hospice athome caseload. As well as deliveringconsultant-led clinical services, the postholder will have the opportunity to lead on service development projects and participate in medical education.
The post holderwill join a dynamicand supportive consultant team:
Dr Nick Dando (Medical Director WTE) Dr ClareStatham (Lead Consultant in Palliative Medicine - WTE), Dr Denise Forth (Consultant in Palliative Medicine WTE), Dr Jenny Skinner ( WTE split between Thames Hospice at Home and WexhamPark Hospital (WPH)).
Close working relationship with Lisney,Consultant in Palliative Medicine WPH.
Main duties of the job
To provide medicalleadership and supportthe provision of holistic medicalcare to patients on the virtual ward, working closely with othermembers of the medical and wider multi-professional team as per a designated job plan.
To provide medical support to the Community teamthrough advice, caseload reviews and domiciliary visits or outpatientappointments.
To support and supervise medicalstaff (Specialty Doctors,Clinical Fellows, GP trainees and FY2doctors) in their day to day practice in the community.
To participate in weekly Community SpecialistPalliative Care Multidisciplinary Team meetings.
To provide Consultant cover to the inpatientunit when needed as part of wider Consultant cover across the hospice.
The post holderwill participate in the delivery of educational projectsand training programmes at the hospice.
Participate in regularclinical, audit and team meetings
Be encouraged to actively identifyareas for qualityimprovement and audit, and to lead on these projects
Support service improvement projects and auditsin progress, including mentorship of junior doctorsin this region
Be encouraged to identify areas for researchand where possiblelink into researchprojects
