An exciting opportunity has arisen within the CURE Project to join the team. Applications are invited for the above post within the successful Tobacco Addiction Treatment (CURE) Team based at Leighton Hospital and across community sites.
We require an enthusiastic, motivated and flexible qualified nurse who has an interest in and awareness of smoking-related issues.
The successful candidate will have excellent communication skills and the ability to motivate others. He / She will be able to work autonomously, and also engage with other healthcare teams and professionals.
The post consists of dedicated time to clinical practice, seeing ward patients and other specialist areas. The role involves teaching and training to all levels of both clinical and non-clinical staff, as well as providing advice to multidisciplinary team colleagues in the clinical setting.
Main duties of the job
This role requires the post holder to work within the acute care setting working in partnership with Mid Cheshire NHS Foundation Trust to ensure the smooth and timely enrolment and treatment of patients with tobacco addiction.
The post holder will work with individual patients using a motivational approach and will give expert advice to both patients and to health care staff including all members of the multi-disciplinary team. The post holder will be responsible for coordinating the care and support of all patients accessing Nicotine Replacement Therapy. The post holder will respond to referrals, undertake holistic assessment and coordinate the delivery of the Nicotine Replacement therapy, this includes supporting patient/carers, communication and working closely with acute wards.
The post holder will be fundamental to ensure consistent delivery of the service and maintain communication and a joined up clinical approach across community and secondary care. As the service develops, the referral sources will be broadened.
Job responsibilities
SERVICE DELIVERY
* Undertake comprehensive health care needs assessment of patients, reassessing as appropriate.
* Facilitate patients to take an active role and support in advice/treatment provided.
* Provide a resource of advice and expertise to community patients, their families, carers and other health care professionals as appropriate.
* Implement and monitor systems and processes to ensure that patient needs are comprehensively assessed, appropriately planned, effectively implemented and evaluated according to changing health care needs and varying levels of complexity.
* Implement change and best practice in terms of patient flow, admission and discharge arrangements according to service plans and objectives, by providing clinical expertise, advice and guidance.
* Work collaboratively with the multidisciplinary team/agencies to ensure that patient needs are met, especially in relation to ongoing care needs and discharge arrangements.
* Involve patients, relatives/careers in the delivery of care, providing reassurance and support to assist their understanding and cooperation.
* Recognise and respond appropriately to urgent and emergency situations and ensuring that others do so.
* Collect, collate, and report information, ensuring that accurate EMIS patient records and documentation are maintained.
* Practice in accordance with the NMC Code in order that a safe and quality service is provided.
* Hold motivational conversations with patients who smoke to encourage their participation in the CURE programme.
* Contribute to the organisation of multi-disciplinary, patient centred care.
* Responsible for assessing and recognising emergencies in their specialty. Able to interpret information and take appropriate action and lead others to do so.
* Undertake and promote practice sensitive to the needs of patients and families from multi-cultural backgrounds.
* Demonstrate the mandatory competencies of the Trust, attending all required mandatory updates.
* Participate and assist inpatient wards with discharge planning to support patients' attempts to quit smoking.
