Job summary We are looking for a confident and experienced individual to join within the Staffordshire Reconnect Service. The Reconnect service sits within our wider Health and Justice service, supporting people with needs and vulnerabilities in all aspects of the criminal justice system. Reconnect reaches into 5 of the prisons within Staffordshire, undertaking holistic needs assessments and engagement work in order to support them on release from prison to get their health and social care needs met within the community and reduce the risk of reoffending. Reconnect sits within a national network of reconnect services, and works both within and outside of the prison estate. IOH. Outreach workers work intensively with a caseload of service users under the indirect supervision of a registered practitioner. Supporting service users on release from prison for up to 6 months in order to engage with appropriate services and support to meet their needs. Successful individuals will have excellent engagement skills, be non-judgemental and have the ability to travel across the Staffordshire area Main duties of the job 1. Being proactive in identifying any additional needs or changing needs. The aim of the intervention is to support and empower the individuals to reach identified goals and attend initial appointments with identified service. This will involve the individual in planning and prioritising their needs. 2. Will be responsible for liaising with multiple agencies, criminal justice and other partners. 3. Support and refer people of all ages with a wide range of mental health, learning disability, substance misuse and/or social vulnerabilities. When a person is assessed as having single or multiple vulnerabilities, they will be referred to the appropriate treatment or support service and an appropriate package of care and/or support will be instigated.
