We have an exciting opportunity for a registered professional to lead and contribute to the Home First / Reablement service as part of the Forest, Tewkesbury, Newent & Staunton locality. The main base for this post is Tewkesbury but there will be some travelling between sites.
The successful candidate will join the experienced Forest/TNS ICT multi-disciplinary leadership team and work closely with the County Home First/Reablement Lead and colleagues in all localities and system partner organisations to further extend the service and build on the continued development of the service.

We are seeking candidates with direct experience in the delivery of Home First - Reablement or community rehabilitation and significant experience of working clinically, leading on decision making, and providing supervision and direction as a member of a multi-disciplinary team within a community setting.
Main duties of the job
You will have responsibility for leading and managing the daily delivery of the Home First / Reablement service for Forest/TNS Locality.
You will ensure there is robust, safe clinical governance and financial management of the Service and work with the Community Manager and County Home First/Reablement Lead to ensure that the strategic direction for the service is effectively implemented at an operational level.
You will lead on projects across the service in conjunction with leadership team leads for Forest/TNS Locality Integrated community teams and Senior Managers. With this in mind, you will need to have a strong operational awareness with a proven track record of delivering change in clinical teams, able to think innovatively and have the ability to influence, motivate and inspire others.
You will have proven experience in training and education of staff and students, providing clinical supervision and seeking and maintaining continuing relationships with key partners.

Qualification
* Current registration with HCPC/NMC
* Relevant qualification educated to degree (level 6) (or diploma equivalent)
* In possession of/working towards a Masters level qualification or able to demonstrate equivalent relevant clinical/managerial/leadership experience
* Recent study/training/ CPD linked to leadership skills
* Practice educator - teaching experience
* UK driving licence

* Experience/demonstrates understanding of working within rehabilitation
* Experience of providing professional and clinical supervision to staff at a variety of levels
* Experience of identifying and managing risk, planning appropriate interventions to mitigate identified risk, and working with escalation processes
* Understanding/knowledge of evidence-based outcomes within community therapy teams
* Extensive post-registration experience in the requisite discipline or evidence of transferable skills
* Able to demonstrate a sound understanding of MDT service delivery for patients
