The Discharge Lounge is offering an exciting opportunity to join the team as a permanent Band 5 Registered Nurse.
The successful candidate will have experience of acute hospital care and a knowledge of patient discharge processes.
The role requires the individual to have enthusiasm and a passion to drive the service forward and meet service demands. They should have effective communication skills and an ability to support all the staff within the team to deliver high quality safe patient care.
The Discharge Lounge supports all areas within the Trust with an emphasis on maintaining timely and safe patient flow out of the Hospital.
Working Pattern: 37.5 hours per week during the hours of 07.30 - 20.00 Monday to Friday and 10.00 - 16.00 weekends.
Main duties of the job
To assist and facilitate Patient Flow for the wards and patients in a timely discharge process
To provide clinical leadership within the ward area
To provide and maintain a high standard of evidence based, patient centred care
To demonstrate good interpersonal skills and to develop practical skills required within the specialty, whist reflecting the philosophy of care
To deliver patient care daily, using the systems in place in the department for a multi-disciplinary approach to patient-centred care delivery, which builds on the philosophy of greater involvement of patients and their relatives/carers by using the Trusts patient and public involvement strategy
To provide direction, supervision and control to team members in order that they provide patient care and services which are appropriate to the individual needs of the patient and the ward/unit
To provide a friendly, effective reception, service to patients
Support and assist Discharge Lounge staff in the day-to-day management and contribute to the care environment
