We have an exciting opportunity within Grays PCN for an enthusiastic, highly motivated Clinical Pharmacist to join our team.
Newly qualified pharmacists and experienced pharmacists who want to join the Primary Care Sector are more than welcome to apply. The role is full-time but we can accommodate for the right candidate with a part-time offer. Self-employed is the preferred option but we can, again, accommodate by offering the post to the right candidate on an employed basis. The successful candidate will be required to work FACE TO FACE at the practices.
Grays PCN is a collaboration of 10 GP practices serving approximately 73,000 patients. Four of the ten practices are training practices. The PCN practices all use SystmOne, for which training will be provided, if required.
The successful candidate will be joining a diverse, experienced and supportive team, made up of GPs, practice nurses, paramedics, clinical pharmacists, first contact physiotherapists, health care assistants, social prescribing link workers, care coordinators and skilled administrators.
The PCN member surgeries & staff meet monthly, and are led in their work by a clinical director and supported by a dedicated PCN operations manager.
Main duties of the job
This role will involve working as part of the general practice team to provide specialist advice to patients and medication reviews to proactively manage people with complex polypharmacy, particularly the elderly and those with multiple conditions.
Work in general practice will also involve medicines reconciliation, management of prescribing queries and acute medication issues following hospital discharges, and the development and implementation of safe systems for medication ordering and prescribing in the context of improving safety and reducing waste.

Job responsibilities
* Clinically assessing and treating patients, using expert knowledge of medicines, for specific disease areas.
* Providing specialist expertise in the use of medicines and medicine information (i.e. side-effects, dosage) across the practice.
* Providing medication reviews, and health education advice, to patients at the designated practices by face-to-face, telephone or notes-based review, as agreed with the practice.
* Providing leadership on person-centred medicines optimisation and quality improvement.
* Acting as a key contact in the practices, for all medication related queries.
* Supporting improvements in clinical care, through practice-based audits and implementing change.
