Job responsibilities Long Term Conditions & Health Checks (LD and SMI) Provide a range of clinical diagnostic skills in order to provide annual health checks, recording onto the patients record and providing onward care and information. Homebound Patients Support the organisation, coordination, and delivery of Care to patients that are homebound. Provide a full health check including diagnostics and phlebotomy. Provide Patients with Immunisations both seasonal and long term. Attend the MDTs, recording and following up actions within defined timescales agreed during the meeting. Work under the direction of the PCN Clinical Director and Homebound team, in patients homes carrying our diagnostic procedures on patients. Work with members of the primary care teams, to develop and implement proactive and reactive care and ensure data collection systems are completed accurate and in a timely matter. Raise awareness within the PCN to shared decision making and decision support tools.
