Job Overview Diana Children s Continuing Care Team provide respite/healthcare support to children/young people with complex healthcare needs, including life limiting/life threatening conditions in a variety of settings. Primary role of the healthcare worker is to provide support in the home; however, you may be required to work in additional settings such as in schools, Child and Family Support Service, Phlebotomy, and children outpatient clinics. To provide effective, evidence based general and clinically specific care for patients in the community, under the indirect or remote supervision of registered practitioners. Work in close collaboration with parents/carers to provide care to children and young people in accordance with the individual care plan to meet nursing and therapy needs. Contribute to the assessment of children s and young people s health and wellbeing alongside qualified staff. Monitor and evaluate children s young people s health and care management.
