Main duties of the job
We are looking for a motivated, friendly, clinically astute experienced Practitioner to join our existing team to support delivering the highest standards of patient care through face-to-face and telephone appointments for patients presenting with a wide range of minor illnesses, under the supervision of our Same Day Access GP, as well as managing and caring for the chronic disease requirements of our patient population.

Job responsibilities
A qualified registered nurse who is responsible for attending to patients in managing their continuing medical and nursing needs. The role will involve supporting patients registered to John Hampden Surgery within the community setting. Where a nurse is recruited, the post holder will adhere to the NMC code of conduct.
1. Diagnosing and treating patients
2. Review, advise and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary or secondary health care team
3. Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations
4. Patients should be treated with evidence-based health interventions in accordance with local policy and guidelines
5. Providing assessment, screening treatment services and health education advice, including:
* Dietary advice
* Blood pressure checks
* Urinalysis
* Health screening
* Dressings
* ECG
* Phlebotomy
* Routine injections
* Contraception
* Routine immunisations and vaccinations
* Removal of sutures/clips
* Baby immunisations
* First registration checks / health surveillance / general health advice
* Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to other members of the primary and secondary health care team as necessary
* Undertake management reviews to meet Quality and Frameworks target
* Conduct Minor Illness assessment and guidance to patients under the supervision of our same day GP
* Pathological specimens and investigatory procedures
* Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc. Perform investigatory procedures requested by the GPs
* Administration and professional responsibilities
* Participate in the administrative and professional responsibilities of the practice team
* Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
* Ensure the clinical computer system is kept up to date, with accurate details recorded and amended
* Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator
* Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
* Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
* Attend and participate in practice meetings as required
* Restocking and maintenance of clinical areas and consulting rooms
* Supplies and equipment
* Maintenance of equipment and stock relating to patient care
* Report any problems to Practice Manager
* Post holder must be able to drive and hold valid insurance that covers home visiting element of role, at their own cost
* The post holder will be responsible for the upkeep of their own vehicle with the right to claim mileage allowance in line with the practice's travel expense policy

Person Specification
Experience
* Nursing experience within a Primary Care setting, OR
* Experience of working within a team
* Experience with minor illness and acute care
* Experience of dealing with all age groups including children and vulnerable patients
* Experience of working in primary / secondary care setting
* Experience of working in a GP practice of Primary Care Network
Qualifications
* Hold a Qualified Nursing Qualification and be registered with the NMC
* Hold Minor Illness and / or Prescribing qualifications (preferred)
* Qualified in Phlebotomy
