Job summary We are looking for an enthusiastic, skilled and motivated Community Support Worker to join our Quickstart service across Wandsworth and Merton boroughs to deliver high quality, timely care support with an enablement focus to patients in the community. Quickstart service provides rapid response social care services for patients who are at risk of social care breakdown. Without a social care package in place these patients would otherwise be admitted to hospital due to an overriding social care need. Main duties of the job Quickstart works closely with Maximising Independence Team, a dynamic, multi-disciplinary team of physiotherapist, occupational therapist, speech and language therapist and rehab assistants, allowing for joint working with rehabilitation and learning opportunities. The post will provide an opportunity for the successful candidate to develop their clinical skills and knowledge base.
