The CRT supports people who have chronic and disabling mental health problems that may be treatment resistant and who have previously spent long periods in hospital.
The CRT East team is based at Kings Park Hospital, with an element of homeworking factored into the team, and all staff will be provided with appropriate IT equipment.
You will work as part of an integrated multi-disciplinary team that includes Mental Health Practitioners, Occupational Therapists, Psychologists, Psychology Assistants, Support Time Recovery Workers, Employment Specialists, Social Workers, Care Managers, Admin Assistants, Advanced Nurse Practitioners, and Medical Staffing.
What you can expect to be doing:
1. Undertaking comprehensive Care Act assessments focusing on the needs of patients open to the CRT.
2. Supporting and enabling people to minimise dependence and maximise independence through strength-based, self-directed support.
3. Working alongside qualified Social Workers and Occupational Therapists, monitoring and reviewing packages of care to meet individual assessed needs.
4. Managing your own caseload, which will entail visiting residents within their own homes, care home environments, and hospitals.
5. Collaborating closely with health partners and other provider organisations.
What we’re looking for:
1. Recent experience of working within Adult Social Care or a similar agency, involving direct contact with members of the public.
2. An understanding of the social and legal context within which social care services are delivered.
3. Excellent communication skills, advocating for adults to assist them in achieving their outcomes, including signposting and providing information and advice.
