Job overview
This is a 12-month fixed-term contract for a practitioner (RGN or AHP) to join the Complex Care Support Team, supporting children with medical complexities across the Trust. The role will address non-medical barriers to discharge and requires an enthusiastic, innovative, and problem-solving applicant.
Main duties of the job
* Be a team member within Alder Hey’s Complex Care Support Team, contributing to clinical work including accepting, prioritising and assessing referrals of children and families.
* Provide clinical care co-ordination of complex discharge pathways while working collaboratively within a wider multi-organisational team.
* Work in partnership with health professionals, parents, carers, and other service providers to ensure a child-centred approach to discharge planning.
* Provide consultation to professionals from other services and participate in service development.
* Provide cover to other areas of service as necessary to support team members.
* Provide clinical management and oversight of complex care arrangements for patients preparing for adulthood.
Detailed job description and main responsibilities
Clinical
* Function as an autonomous senior member of the MDT using experience and knowledge of specialist pathways.
* Complete comprehensive assessments for patients with complex health needs.
* Interpret assessment outcomes into meaningful formulations for patients, their carers, and the MDT.
* Recognise and respond to safeguarding issues, adhering to Alder Hey policy.
* Communicate knowledge of therapeutic processes and care plans to clients and families.
* Utilise analytical skills to provide a comprehensive assessment considering various factors affecting the child.
* Manage cases of some severity and complexity, coordinating multi-disciplinary inputs.
* Adopt a client-centred approach to care, collaborating with children, parents, and education staff.
* Participate in team and multi-disciplinary meetings, representing outcomes of assessments as needed.
Person specification
Essential and Desirable
* Relevant professional registration (e.g., RGN, Social Worker, OT).
* Experience working with children with medical complexities.
* Able to build strong collaborative relationships with clinical leaders and staff groups.
* Proven knowledge of safeguarding procedures.
