Outreach Practitioner – Rapid Response Team (Band 6/7 Depending on Experience)
Part-Time – 15 Hours Per Week

The Rapid Response Team at Frimley Park Hospital is seeking a highly skilled and clinically motivated Outreach Practitioner to join their team.
Key Responsibilities
1. Provide clinical support for patients experiencing or at risk of clinical deterioration, life-threatening events, or cardiac arrest.
2. Respond promptly to medical emergencies, ensuring timely intervention and management.
3. Contribute to the development, implementation, and evaluation of deteriorating patient care protocols and resuscitation policies.
4. Collaborate with the multi-professional team to ensure high-quality patient care.
5. Demonstrate strong clinical judgment and decision-making skills in acute situations.
Required Skills & Experience
1. Extensive experience in managing medical emergencies and deteriorating patients.
2. Proficiency in resuscitation procedures and emergency response.
3. Ability to develop and implement clinical policies related to patient deterioration and cardiac arrest management.
4. Excellent communication skills, fostering effective team collaboration and patient care.

This role is ideal for a motivated practitioner with a strong background in acute and emergency care, seeking a part-time opportunity to make a significant impact on patient safety and clinical outcomes.
This is a clinically focused and educational role at Frimley Health.
The post holder will support the Band 7 in the day-to-day provision of the Critical care Outreach services across Frimley Health.
This role will require the post holder to work weekends, evenings, and night shifts as required for service delivery.
Working within the Rapid Response Team providing nursing intervention with appropriate assessment and management of the deteriorating patient and those at risk of deteriorating in Frimley Health.
