Bayada Home Health Care - Admissions Coordinator

Responsibilities:
* Coordinate patient pre-admission activities and manage referral processing from physicians, hospitals, and community sources.
* Collaborate with referral sources, insurance companies, patients, and families to ensure seamless service delivery.
* Process patient referrals, coordinate admissions, and work closely with hospice operations and sales teams.
* Enter patient admission data accurately once all hospice requirements are met.
* Work with billing, clinical information, and referral sources to build effective relationships for patient benefit.
* Contact insurance providers for eligibility and authorization, confirming hospice service line details.
Qualifications:
* High School diploma or GED required; Bachelor’s degree preferred.
* At least two years of experience in hospice or related healthcare fields.
* Experience in medical office, customer service, intake, or inside sales preferred.
* LPN/LV certification is a plus but not mandatory.
* Proficiency in Microsoft Office Suite (Excel, Teams).
* Positive attitude, eager to learn, and excellent communication skills.
