Job summary An exciting opportunity has arisen within the Podiatry Service for a professional and dedicated Clinical Care and Activity Coordinator to join our friendly and dynamic team. This exciting new role will provide proactive high quality systems administrative support and ensure efficient and effective management of clinical activity through robust clinical rota and waiting list coordination and management. This role will also coordinate multidisciplinary care with healthcare professionals. We are looking to recruit an organised and enthusiastic individual with extensive SystmOne experience who can support the Podiatry Service to transition to ways of working. Main duties of the job Proactively assist in the efficient and effective management of clinical ledgers/rotas and waiting lists in line with local requirements and standards to ensure clinical capacity is optimised and workload managed in a timely, accurate, and effective manner. Work alongside Senior Leaders to ensure that clinical ledgers/rotas reflect demand and clinical requirements and are utilised to maximise available resources to support patient care. Proactively manage clinical appointment ledgers/rotas and waiting lists, reporting key performance indicators, managing tasks and referrals within SystmOne, liaising with patients, families, carers and health professionals, managing the administrative aspects of the Podiatry Clinical Advice Email and escalating to the appropriate clinician, and working with Operational and Clinical Leads to build a robust framework with SystmOne to support clinical staff to optimise their clinical activity. Work with Podiatrists to deliver care coordination activities for those patients who require input from multiple services, such as arranging multidisciplinary meetings and 'Team Around The Patient' meetings. Support the Podiatry Service through a period of change in which it is developing new administration and care coordination processes to support care delivery. About us The Sheffield Teaching Hospitals Podiatry Service is composed of three Teams: Empowerment (Triage and First Contact), Community Podiatry (Foot Protection Service, At-Risk Foot Team, Nail Surgery Team, Rheumatology Podiatry) and the Acute Team (Diabetic Foot Multidisciplinary Team, Vascular Podiatry, Renal Podiatry, F-Scan and Offloading Clinics).
