NURSE PRACTITIONER

Key Responsibilities:
Patient Assessment & Diagnosis:
· Conduct comprehensive health assessments for patients, including physical examinations, history taking, and diagnostic tests.
· Formulate differential diagnoses based on clinical findings and patient history.
· Order, perform, and interpret diagnostic tests and imaging as required.
Treatment & Management:
· Develop, implement, and evaluate evidence-based treatment plans.
· Prescribe medications, therapies, and other interventions within the scope of practice as regulated by the College of Nurses of Ontario (CNO).
· Provide education to patients and families regarding management of health conditions, treatment plans, and wellness goals.
Health Promotion & Disease Prevention:
* Offer guidance and resources for disease prevention, health promotion, and lifestyle changes.
* Conduct screenings and immunizations for individuals across all age groups.
* Address mental health needs, offering counseling or referrals as appropriate.
Collaboration & Communication:
* Collaborate with other healthcare professionals to coordinate comprehensive care plans.
* Provide consultation, education, and mentorship to other healthcare team members as needed.
* Communicate effectively with patients, families, and the healthcare team to ensure continuity of care.
Documentation & Record-Keeping:
* Maintain accurate, up-to-date patient records using electronic medical records (EMR) systems.
* Document patient interactions, diagnoses, treatment plans, and follow-up care in compliance with legal and ethical standards.
Professional Development & Continuous Learning:
* Stay current with the latest clinical guidelines, best practices, and medical research relevant to the role.
* Attend professional development opportunities and maintain certification/licensure requirements as set by the College of Nurses of Ontario.
Qualifications and Experience:
* Master of Science in Nursing (MScN).
* Registration without restrictions in the Extended Class with the College of Nurses of Ontario.
* Must be in good standing with College of Nurses of Ontario (CNO).
* Minimum of 3 years of experience as a Nurse Practitioner, preferably in a family medicine or primary care setting.
* Ability to work independently as well as part of a collaborative healthcare team.
* Strong organizational and time-management skills.
* Active BLS certification is required; or the willingness to obtain.
Knowledge, Skills and Abilities:
* Ability to make sound clinical decisions and manage a diverse patient population.
* Ability to provide compassionate and patient-centered care, building rapport and trust with patients and their families.
* Excellent verbal and written communication skills, with the ability to explain complex medical information in an understandable manner.
* Strong analytical and critical thinking skills, able to assess situations quickly, make decisions, and provide effective solutions.
* Highly detail-oriented in documentation, patient care plans, and medical procedures to ensure accuracy and safety.
* Ability to work well in a team environment, collaborating with other healthcare professionals to deliver comprehensive patient care.
* Flexibility to adapt to changes in practice demands.
Position Details:
This is a 1-year part-time contract position (maternity leave) with the possibility of adjustment to schedule based on candidate and clinic needs.
