District Nursing Team Lead for Care Home Service

Main duties of the job
* Lead the daily operations of a team of community nurses and HCAs.
* Ensure safe and effective allocation of work across all teams.
* Directly line manage and support a team of Band 6 nurses, with oversight of the full Nursing team.
* Oversee the budget, including processing invoices and expenses.
* Ensure safe staffing levels and maintain the E-roster system.
* Facilitate regular team meetings and participate in MDT meetings.
* Respond to incidents and address any safeguarding concerns.
* Provide caseload supervision.
* Collaborate regularly with other healthcare professionals, including GPs, physiotherapists, occupational therapists, social workers, and care providers.
* Monitor and support staff wellbeing.
* Provide clinical support to the team and student nurses when needed.
* Ensure staff maintain competencies and receive ongoing education and training.

Person Specification
Knowledge
* Knowledge of the Principles of Governance, particularly in relation to their service area.
* Knowledge of National Documents relating to District Nursing and Primary Care.
* Knowledge of working between Acute and Primary Care settings.
* Knowledge of Chronic Disease Management.
* Knowledge of change management and service development.
* Knowledge of clinical audit.
Qualifications
* Registered level 1 Nurse.
* Specialist qualification in District Nursing or postgraduate diploma/degree level study related to health.
* Teaching/Assessing course such as mentorship module or equivalent experience.
* Recognised Management qualification or courses e.g., ILM course, or leadership, management modules, core management study days, in-house courses (HR, budgets, appraisals, staff performance, sickness).
* Ability to travel across the city.
Experience
* At least 3 years experience in a senior clinical role with experience of managing a team of staff including caseload management.
* Proven skills in leading, managing and developing teams.
* Experience of inter-agency and partnership working.
* Experience in challenging clinical practice and leading in the development of new pathways of care and delivering best practice.
* Experience of community working, or an understanding of community working.
* Relevant clinical experience and competence, including knowledge of community nursing skills.
* Experience in carrying out clinical audit and risk assessment.
* Experience of leading projects.
