* Promote the health and wellbeing of clients and encourage positive attitudes and practices.
* Provide support for clients who because of their complex health needs, cannot access or having difficulty accessing Primary and Hospital healthcare.
* To liaise and work jointly with the Community Nurses and other professionals in the team as required to meet the clients' overall health needs.

Under the supervision of the Senior Community Nurse assist in the provision of a high quality individualised and person-centered service for clients with complex health needs in addition to their learning disabilities. The ideal candidate will be caring, empathetic and confident to work in a practical role working directly with clients.
