A patient’s recovery can be impacted by staying in hospital longer than is necessary, so having the right care plan in place to support the person to leave hospital at the earliest and safest time for them is crucial.
The Acute Trust Case Managers support the effective management of patient discharge from hospital and play a pivotal role in supporting ward-based colleagues with patient discharge plans.
The Acute Trust Case Managers work with all members of the newly formed Transfer of Care Hub (ToCH) based at the BRI, including social workers, Sirona CToCH (Community Transfer of Care Hub) team, and community therapists and will be encouraged to participate and share learning with colleagues across the team.
We are looking for Registered Nurses or AHPs (Allied Health Professionals) to join our team.
The Home First Team operates across all adult inpatient areas at the BRI and Weston Hospital.
You will:
* Use your expert knowledge to coordinate, facilitate, and move forward all patient discharges on your wards.
* Be responsible for a defined caseload working with wards on a rotational basis. You will gain experience in managing patients who have discharges that are “delayed”, “complex” and also work with the admission areas (e.g. Emergency Department) to prevent admission to the hospital.
* Support and complete initial assessments for Continuing Healthcare Fast Track Assessments (CHCFT) and referral to the Community Transfer of Care Hub (CToCH) for pathway beds.
* Liaise with the wider MDT, Local Authorities, other Trusts, CToCH, and Integrated Care Bureau (ICB) through meetings and the Trust Transfer of Care Hub (ToCH).
* Ensure local and national policies and guidelines are adhered to, ensuring that the patient and their families are at the heart of everything we do by communicating and supporting them throughout the discharge process.
* Build key relationships with the ward and therapy teams to ensure good communication and collaborative working relationships to facilitate a timely safe discharge.
