Business and Service improvements responsibilities The core objective of the PCN Digital and Transformation Lead role is to support the PCN and its practices to deliver ongoing improvement to services. The role will require experience in business and service improvement. The role will require an understanding of general practice systems and processes, including broad familiarity with general practice digital systems and how they can be used to deliver improvements. To work alongside practices to understand current challenges to effective service delivery and identify solutions, including existing best practice within the PCN. Drive the planning and prioritisation of changes that will improve delivery of care and improve patient and staff experience in the practices within the PCN. Ensure all change is ultimately focussed on improving services for patients. Build relationships across the PCN to develop consensus for improvements and to ensure business and clinical staff are fully engaged throughout the process of selection, implementation and deployment of change. Link with the ICS and other partners to gain buy-in for transformation and to support alignment with broader strategy and improvement approaches. Provide hands-on capacity and support to practices in implementing and sustaining agreed changes. Support staff to identify and participate in relevant training and professional development opportunities that will support achievement of agreed improvements. Support practices and staff to effectively use existing and new technology systems and products to enable improvement. Finance and Governance Lead on, and hold operational responsibility for, the management of finances in the PCN supported by the Clinical Director and PCN administrator. Maximise income potential and ensure equitable distribution of money to practices. Monitor and check the accuracy of the submissions for the DES and contractual requirements. Ensure that all claims are accurate and submitted in good time and payment is made and received. Act as point of contact and lead liaison with PCN accountants and bookkeepers. Maintain and update the PCN business continuity plan in alignment with core GP practice plans. PCN Board Working with the Clinical Director, set the PCN board agenda and circulate in a timely manner. Support the Board and practice teams to ensure understanding of the current PCN guidance from NHS England and the Cheshire & Mersey Integrated Care Board (CMICB), keeping them informed of anything new or significant. Coordinate and liaise with the Extended Board members including arrangements for meeting attendance. Act as a point of contact for all stakeholders (PPG, Healthwatch, ICCT). Ensure Board meetings are run in accordance with the Terms of Reference of the PCN Board. Approve the board minutes before circulating to the Board members. As required, to minute the PCN Board meeting, and other meetings. Digital and Transformation (including information and data management) Lead on change management for the PCN, using formal and informal change methods. Identify digital technology that can improve service delivery and patient outcomes for the PCN. Take a data driven approach, looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made. Develop high-level understanding of capacity and access data sets and technology. Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges. Use data to track and assess impact of changes made to operational processes, and to track trends and developments over time. This will include monitoring of ARRS staff activity data. Contractual Provide the PCN Board and PCN Practice Managers with current information about PCN service specifications. Identify, scope and cost ad-hoc PCN activities for new work streams. Lead on PCN-wide projects identified as per the PCN DES e.g. workforce planning, capacity and access development and digital transformation. Lead on all subcontracting arrangements for PCN services (e.g. First Contact Physiotherapy, Enhanced Access, Social Prescribing). Keep abreast of all PCN contract developments and communicate to PCN Board. Liaise with ICB to ensure all contractual obligations are met and understood. Human Resources Support the recruitment and retention of directly employed ARRS staff. Lead on workforce planning, with the PCN clinical director. Attend an annual appraisal with the PCN clinical director. Manage own CPD to maintain relevant understanding and knowledge in support of the role. Ensure contractual documentation is maintained in accordance with core practice requirements. Communication and Engagement Develop strong relationships and engage with a wide range of stakeholders across the locality to ensure collaborative approaches to service development and delivery. o Clinical Directors o PCN Board o PCN Practice Managers o ICB o Voluntary sector, Local Authority and Secondary Care organisations. In addition to the main duties of the role, the Digital & Transformation Lead may be requested to: Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner. Duties may vary from time to time without changing the general character of the post or the level of responsibility.
