Job summary
The Community Nursing Service within NorthEast Lincolnshire provide nursing care and support to patients, families andcarers in a variety of community settings supporting patients to remain intheir own homes, preventing hospital admission and facilitation of earlyhospital discharges and managing end of life care.
Our Community Nursing staff workclosely and in partnership with Primary Care Networks, Secondary Care, SocialCare, Specialist Nurse and other healthcare staff to provide a quality servicepersonalised to meet individual needs. The service supports and assistspatients to manage ill health and long term conditions including elements ofdisability, thus allowing them to have the best possible quality of life, whilemaintaining dignity and respect.

Job title: Community Staff Nurse
Fixed Term - 12 Months: 30 Hours Per Week
Base: Cleethorpes Primary Care Centre
You will berequired to work a rota pattern which includes weekends and Bank Holidays
Salary Range 5 £29,734 - £34,474 pro rata per annum
We are looking toappoint a qualified Registered Adult Nurses 1st Level with previous communityexperience and additional post registration nursing qualifications Tissue Viability, palliative care, nurse prescriber etc.
You willhave the ability to maintain clear and accurate records using informationtechnology and will have a proven ability to work on your own initiative aswell as part of a team. The post involves the day to day management of anunpredictable caseload, some of which will be complex in nature and thereforeeffective organisational and prioritisation skills are essential.
The post requires you to transport yourself and equipmentacross locations within North East Lincolnshire and to deadlines. Therefore a flexible approachto your working arrangements is essential.
