To identify Carers coming into the CMHT and ensure that they are offered a Carers Assessment as per
the Section 75 delegated responsibility from the Local Authority.
To lead on completion and inputting of carers assessments, following the process through to payment where identified and keeping a track of those requiring annual re-assessment.
To ensure that the role of unpaid carers is recognised, supported and valued. To ensure that carers are well informed, supported with their individual health and well-being needs and involved in decision making about care in relation to their caring role.
To provide carers with information and advice about services both within and external to the organisation, that contributes to their caring role and increases their knowledge regarding care and treatment processes. To signpost and facilitate carers access to a range of support services where appropriate.
