The Team:
The PD Nurse will work alongside the Consultant Nephrologist, Nurse Unit Manager, Renal nursing staff, Chronic Kidney Disease CNC, Vascular Surgical Team, Specialist Clinic staff, Radiology team and other multidisciplinary team members at the Launceston General Hospital and Royal Hobart Hospital. This service will support patients attending the Renal Unit facilities and satellite units of the northern region who are undergoing dialysis or about to start dialysis. All our services work closely together to focus on delivering excellence in healthcare to achieve improved outcomes for our patients.
Launceston General Hospital is one of Tasmania’s largest hospitals, alongside the Royal Hobart Hospital. Launceston General Hospital is a major clinical teaching and research centre, LGH works closely with the University of Tasmania and other institutions.
The Role:
* Lead case management in the clinical management and ongoing care for patients undertaking renal replacement home therapies and involve patients and their families/carers in the planning and implementation of care.
* Provide leadership in the review, development and evaluation of policies, procedures, protocols and best practice related to home based renal replacement therapies and related patient/client care and make recommendations for alternative approaches to the NUM and/or Clinical Nurse Consultant.
* In collaboration with members of the multidisciplinary team plan, implement, evaluate, and support complex case models and interact effectively with all stakeholders to facilitate the provision of optimum patient/client care.
Details of appointment:
Permanent part time, day worker position, working up to 48 hours per fortnight, commencing as soon as we can onboard you.
*notwithstanding hours to be negotiated with the successful applicant.
Salary: $106,570 to $112,440 pro rata per annum. Our Employer 11.5% superannuation contribution is on top of this amount.
* Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.
Eligibility:
* Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.
* Current Driver’s Licence.
