This is an exciting opportunity to work for Derbyshire Dales PrimaryCare Network (DDPCN) who work closely and in partnership with the 8 GP Practicesthat are situated in the north and central areas of the beautiful DerbyshireDales.
DDPCN is looking to recruit a full time Pharmacy Technician to work at one of our stakeholder practices ,namely ,Peak and Dales Medical Partnership. This is a GP Practice operating from locations in Bakewell and Tideswell.
Job share can be considered for the successful applicants providing theoverall working time hours are met.
You will be working with and led by our Lead Clinical Pharmacist, ClinicalDirector and PCN Management Team.
Daily you will be engaged by the Practice GPs and wider clinical teamswithin the setting .
Your goal will be to providehigh-quality, integrated care for our patient community.
Main duties of the job
Job responsibilities
Clinical
- Undertake patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients.
- Carry out medicines' optimisation tasks including effective medicineadministration (e.g., checking inhaler technique), supporting medicationreviews and medicines reconciliation. Where required, utilise consultationskills to work in partnership with patients to ensure they use their medicineseffectively.
- As determined by the Practice and PCN, support medication reviewsand medicines reconciliation for new care home patients and synchronisingmedicines for patient transfers between care settings, linking with localcommunity pharmacies
- Support the clinical pharmacist to deliver structured medicationreviews (SMRs) i.e., organise necessary monitoring tests prior to SMR
- Provide expertise to address both the public health and social careneeds of patients, including lifestyle advice, service information, and help intackling local health inequalities.
- Manage shared care protocols and liaise with clinical pharmacist formore complex patients.
- Support initiatives for antimicrobial stewardship to reduceinappropriate antibiotic prescribing locally.

Peak and Dales Medical Partnership are based in the heart of Bakewell town centre and have a branch site at Tideswell. The practice also serves patients living in the many picturesque villages surrounding Bakewell and Tideswell .
There is an excellent road infrastructure with access to the M1 Motorway, Sheffield, Derby, towards Manchester and Staffordshire which make the job easily commutable from these areas .
You will be joining a friendly, forward thinking practice who work in partnership with the Derbyshire Dales PCN .
The PCN also supports 7 other practices in the area, several of which have experienced Pharmacy Technicians embedded as part of the practice team .
There are circa 50,000 patients within the PCN area receiving high quality patient focused care.
* There is an ethos amongst the practices to work collectively to deliver the Extended Access provision.

Job responsibilities
Support the Practice teams to ensure efficient medicines optimisationprocesses are being followed.
- Implement efficient ordering processes and reduce medication wastage.
- Provide training and support on the legal, safe, and secure handlingof medicines, including the implementation of the Electronic PrescriptionService (EPS).
- Promotion of Electronic Repeat Dispensing (eRD) and online ordering
- Develop relationships with other pharmacy professionals and members ofthe multidisciplinary team to support integration across health and social careincluding primary care, community pharmacy, secondary care, and mental health.
- Support practice reception teams in streaming general prescriptionrequests, so as to allow GPs and clinical pharmacists to review the moreclinically complex requests.
- Support the implementation of national prescribing policies andguidance within the practice, care homes and other primary care settings.
- Support the Practice to deliver on QIPP agenda, QOF and locallycommissioned IIF enhanced services
- Support the Practice in reviewing and developing practice policies forCQC requirements
Person Specification
Experience
* Good clinical pharmacy knowledge including terminology
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing
* Knowledge and understanding of pharmacy law and ethics and current legislation
* Has attention to detail, able to work accurately, identifying errors quickly and easily
* Able to effectively manage allocated resources
* Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines
* Able to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to draw logical solutions to problems.
