Acting within their professional boundaries, the post holder is a qualified pharmacist that will provide advice and guidance as part of a multi-disciplinary team for our pre hospital care operations.
The post holder will work with and alongside the general team in a leadership role in supporting further integration of practice with the wider healthcare teams to help improve patient outcomes.

Job responsibilities
Administration / Communication
* To work with the relevant Lead Clinicians to agree a definitive list of minor illness conditions that will form part of the Pharmacists workflow in the longer term
* Communicating any important prescribing messages to practice staff
Audit, Policy and Review
* Reviewing the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews
* Monitoring practice and make recommendations
* Auditing compliance against NICE, JRCALC guidance
Education and Training
* Providing education and training to the healthcare team regarding medicines
Person Specification
Qualifications
* Masters degree in Pharmacy (MPharm) or equivalent
* Registered as a pharmacist with the General Pharmaceutical Council (GPhC)
* Independent Prescriber with GPhC (or working towards the qualification)
* Specialist knowledge acquired through postgraduate diploma level or equivalent training / experience in relevant clinical subject
Experience
* Experience of working in a multidisciplinary team
* Experience in primary and/or pre hospital care
