We are looking for a Health & Wellbeing Coach with abackground in supporting individuals to achieve their health and wellbeinggoals.
Wellness coaching'can be defined as the application of life-coaching principlesto health and wellbeing. Our Health and Wellbeing coach willuse skilful communication to support individuals to make life changes throughworking towards their personal goals.

About us
We support our member GPpractices to help them deliver the best possible care to patients on theIsland. We host a team of Social Prescribers, Health and Wellbeing Coaches andCare Co-ordinators under a Primary Care Contract that support our Islandpractices.
We aim to do that byproviding centralised services that enable GPs to enhance their offer ofsupport to patients.
We also provide directsupport to GPs and practice staff to enable them to develop, lead and managetheir practices more effectively and efficiently on a day-day basis. Forexample, providing training and education, mentoring, recruitment support,resources, access to specialist roles and sharing best practice.
We also help bid for andsecure additional funding from NHS England, where it becomes available, to helpsupport new initiatives that help practices to meet their patients care needsand we work to support general practice/primary care, to make sure it has avoice within the wider healthcare system as it develops plans for the futurecare for our population.

Purposeof the role
Health and Wellbeing Coaches workas a key part of the primary care network (PCN) multi-disciplinary team. Theyconsider the whole person in addressing existing issues and encouraging theproactive prevention of new illnesses, promoting lifestyle medicine approaches.Health coaching revolves around using coaching skills to support people withlower levels of patient activation to develop the knowledge, skills, andconfidence to manage their health and wellbeing, whilst increasing theirability to access and utilise community support offers.
The post holder will work inpartnership with their clinical and non-clinical colleagues, management supportand the wider PCN to ensure the role delivers the best possible outcomes forour patients. The role is varied, and may include supporting self-managementeducation, peer support, case management and facilitating group consultations.You will take an approach that is non-judgemental, based on strongcommunication and negotiation skills. You will support personal choice andpositive risk taking, while ensuring that patients understand theaccountability of their own actions and decisions. As a whole your role andskills will support and encourage the prevention of developing further illness,or the deterioration of existing long term conditions.
Keyresponsibilities
1.Take referrals from the wider practice team including GPS, Practice Nurses,Social Prescribers, Clinical Pharmacists and Physiotherapists who will haveidentified patients that meet the criteria for Health and Wellbeing coaches.
2. Coach and motivate patientsthrough multiple sessions to identify their needs, set goals, and support themto implement and achieve their personalised health and wellbeing objectives.This will include sitting in consultation with a patient and providing themwith advice, guidance and a management plan, personalised to their individualneeds. It could include dietetics and healthy eating, lifestyle medicine andgetting active, safe activities and personal coaching and motivation.
3.Ensure all interventions and coaching are designed to empower patients to beactive participants in their own healthcare, empower them to manage their ownhealth and wellbeing, and live independently.
4.Support and deliver group consultations, with clinical and non-clinicalcolleagues, implementing new approaches to health across primary care andsupporting individuals to access peer support.
5. Work with the broader MDT to maximise thesupport available to patients, including the social prescribing team to connectpatients to community-based activities which support them to take increasedcontrol of their health and wellbeing, and working with clinical colleagues toprovide enhanced support to patients being supported through, identifyingthose who would benefit the most from health coaching.
role will require managing andprioritising your own caseload, in accordance with the needs, priorities andany urgent support required by individuals.
7.It is vital that you have a strong awareness and understanding of when it isappropriate or necessary to refer people back to other healthprofessionals/agencies, when what the person needs is beyond the scope of the coachrole when there is a mental health need requiring a qualifiedpractitioner.
8. Work with social prescribing colleagues to drawon and increase the strengths and capacities of local communities, enablinglocal VCSE organisations and community groups to receive social prescribingreferrals. Ensure they are supported, have basic safeguarding processes forvulnerable individuals and can provide opportunities for the person to developfriendships, a sense of belonging, and build knowledge, skills and confidence.
9. Work together with all local partners tocollectively ensure that local VCSE organisations and community groups aresustainable and that community assets are nurtured, by making them aware ofsmall grants or micro-commissioning if available, including providing supportto set up new community groups and services, where gaps are identified in localprovision.
