Clinical Responsibilities 1. Conduct triage of all incoming AccuRx submissions, determining the most appropriate next steps for each patient. 2. Where clinically appropriate, manage patient cases via telephone consultations, providing timely and effective medical advice. 3. Allocate face-to-face appointments for patients requiring in-person assessment, ensuring they see the most appropriate clinician e.g., Urgent Care Practitioner, Advanced Nurse Prescriber, and Nursing Team. 4. Work in collaboration with the Duty Manager and Reception Leads to streamline triage processes, ensuring timely and efficient patient care. 5. Maintain accurate and comprehensive clinical records of all triage and consultation activities. 6. Prescribe medications in accordance with Practice policy and NHS England Formulary where clinically indicated. 7. Provide health education and lifestyle advice to patients, supporting preventative healthcare measures. 8. Participate in clinical meetings, case reviews, and quality improvement initiatives to enhance service delivery. 9. Respond to medical problems presented by temporary residents. 10. Conduct long-term condition reviews as required, contributing to the management of chronic medical conditions. 11. Ensure compliance with national and local clinical guidelines in all aspects of patient care. 12. Participate in training plans of GP register, medical and nursing students as required. 13. To complete insurance, DSS, DVLA and other reports relating to patients under your care. 14. In general the post holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care and any other duties reasonably required by the Practice. 15. This job description is not exhaustive. We anticipate the role will develop with the suitable candidate and we may amend it as appropriate. Triage-Specific Responsibilities 1. Act as the first point of clinical contact for patients submitting online consultations, determining urgency and appropriate response. Recognise and manage acute conditions, referring patients to emergency services when necessary. Identify patients requiring urgent care and facilitate timely intervention. Support efficient appointment utilisation by ensuring appropriate patient allocation across the clinical team. Use clinical judgement to determine whether further investigation, specialist referral, or alternative treatment pathways are necessary. Book follow-ups, and/or assign tasks to Reception to complete. Assist in developing and refining triage protocols and best practices to improve patient outcomes. Maintaining, Monitoring and Updating Computerised Patient Records 1. Contributing to the development of computer based patient records. 2. Contributing to the summarising of patient records and read coding patient data. 3. Ensure clinical database is updated promptly and accurately as required for practice protocols, QOF, Enhanced Services etc. 4. Respond to alerts on patient records. 5. Appropriate use of read codes as agreed within the practice. 6. Participate in the collection and collation of statistics as required. 7. Participate in audits of clinical work and patient satisfaction.
