Main duties of the job The post holder will be responsible for the assessment and management and care coordination of service users who have a variety of complex needs, diverse presentations, and a range of perinatal mental health conditions. In collaboration with the Service User and their families, the post holder will facilitate formulation which then informs a Recovery and Wellbeing focussed Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary. Deliver evidenced based Clinical and Psychologically based Interventions. Coordinate Treatment Planning & Interventions, including reviews and Discharge Planning. Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Pathway Manager.
