This is an exciting opportunity for an Advanced Nurse Practitioner to work at French Weir Health Centre. The successful applicant will enhance the established team of clinical support clinicians, comprising Advanced Nurse Practitioners and Emergency Care Practitioners. We welcome applications from Advanced Nurse Practitioners from a secondary care environment as we offer supervision and mentorship for skills and knowledge to seamlessly integrate the candidate into primary care. Study leave in addition to annual leave is offered for continuous professional development working within the four pillars of advanced nurse practice.
We are seeking a dedicated and experienced Advanced Nurse Practitioner, who will be a non-medical prescriber to join the team.
The ideal candidate will be a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
Up to 28 hours per week, over 3 days, two days finishing at 8.00 pm.
Main duties of the job
* Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
* Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
* Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.
* Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
* Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
* Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
* Work with patients in order to support compliance with and adherence to prescribed treatments, provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
* Support and manage health needs of women presenting for family planning, menopause or sexual health consultation.

Job responsibilities
The Advanced Nurse Practitioner (ANP) will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The Advanced Nurse Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
The postholder will use skills, knowledge and competencies as a qualified Advanced Nurse Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.
Job responsibilities:
Clinical Nursing Practice
* Work as part of a multi-disciplinary team to deliver safe and high quality care.
* Carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
Delivery of care in line with NSF, NICE guidelines and evidence-based care.
* Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.
* Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
* Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.
Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
* Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.
* Refer patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.
Care Management
Communicate confidential and sensitive information to patients, relatives and carers in relation to their condition.
Provide highly specialised care advice beyond the scope of normal Nursing practice e.g. care treatment plans, making referrals as appropriate (including fast track referrals).
Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme, National benchmarking.
Monitor and meet care outcomes against standards and recommend or initiate changes as necessary.
Health Improvement
Management of health screening programmes and ensures other team members are aware of health priorities and screening objectives and programmes.

Qualifications
* RGN / RN With current NMC registration
* Independent prescriber
* Completed Nurse Practitioner (Primary Care) Post Graduate Diploma level
Experience
* Minimum of 2 years experience post-registration experience of working in primary / community / acute health care setting
* Able to analyse and interpret prescribing data
* Able to manage acute illnesses, long term conditions and minor ailments
* Refer patients independently as appropriate
* Interprets a range of diagnostic tests and routine clinical procedures
* Understanding and experience of inter-agency working across health groups and networks
* Has provided effective health education and health promotion to patients presenting with a range of health needs
* Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines
* Excellent understanding of data protection and confidentiality issues
* Able to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to come up with solutions
* Excellent verbal and written communication skills with team members, patients, carers, and other healthcare professionals whilst recognising peoples needs for alternative methods of communication
* Has attention to detail, able to work accurately, identifying errors quickly and easily
