Two roles available - one full-time and one part-time Location: Peace Hospice, Watford Salary: £46,148 - £52,809 per annum (pro rata) Closing Date : 18 November 2024 at 12 noon Interview Date : 26 November 2024 The purpose of this role is to be part of a small team of senior nurses who are responsible for the clinical coordination of care for patients and families referred to Rennie Grove Peace. You will therefore need to demonstrate your abilities in the following areas: Being the point of contact for patients, families, clients and professionals who contact the hospice, dealing with all clinical enquiries. Providing clinical oversight and triage for referrals to ensure that patients and family members get the right Rennie Grove Peace service in a timely manner. Developing the Coordination Centre's nursing role to ensure it provides a responsive, evidence based and high quality service to beneficiaries and professionals. Working as a Specialist Palliative Care Clinical Nurse Specialist (CNS) in a different Rennie Grove Peace clinical setting on a monthly basis to maintain and develop skills. The post holder will play an active part in promoting the Rennie Grove Peace service to others including external Health and Social Care professionals and wider members of the public. The post holder will undertake activities such as audits and case review in order to improve the Coordination Centre service. Contributing to the wider strategic development of the hospice and help to translate local and national strategy relating to hospice care and enable Rennie Grove Peace to implement and deliver an effective service. Acting as a Rennie Grove Peace Hospice Care ambassador, actively demonstrating our values and positively promoting the specialist care we provide. What we're looking for: Registered Nurse Degree Level or modules in Palliative Care or another relevant subject Proven palliative care experience IT literate including a good knowledge of Excel, Word, a clinical IT system.
