Job summary The Health Check team of Kent Community Health NHS Trust is commissioned to deliver theNHS Health Check Programme across Kent, working with third party providers including GP'sand Pharmacies. The service aims to provide equitable access for all individuals agedbetween 40 and 74 who are eligible to receive a free NHS Health Check and identifyalternative provision wherever possible.In this way, the service supports individuals in Kent by offering a person-centred approach toraising awareness of current health behaviours, identifying areas ofpotential risk with regard to developing Cardiovascular disease and onward referralfor further investigation and/or lifestyle support to encourage and maintain healthierlives.
