Main duties of the job
As a paramedic within our team, you’ll play a key role in delivering exceptional patient care. Your responsibilities will include:
* Assessing, diagnosing, and managing a range of acute and chronic conditions.
* Conducting face-to-face and home visits to provide high-quality clinical care.
* Working collaboratively with the multidisciplinary team to support holistic patient management.
* Contributing to improving access and reducing the workload for GPs.

Job responsibilities
Alongside the multi-disciplinary team (MDT), provide a first point of contact within the practice for patients presenting with undifferentiated and undiagnosed problems. Making use of skills in history taking, physical examination, problem-solving, and clinical decision-making to establish a diagnosis and management plan.
Provide assessment, treatment, and diagnosis at the point of first contact by attending to patients in a variety of clinical or non-clinical settings according to patients' needs.
Undertake home visits in accordance with the relevant protocols.
Make professionally autonomous decisions for which you are accountable.
Refer patients to an alternative care setting or treat and discharge as appropriate.
Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services, PCN services, and to self-care.

Experience
* Experience of working within a multi-disciplinary team.
* Ability to manage and prioritise workload.
* Ability to work flexibly in core hours to meet the needs of the service.
* Proven ability to work autonomously and as part of a team.
* Sound organisational skills.
* Able to professionally and appropriately challenge differing opinions as a patient advocate.
* Sound IT skills in word processing and email.
* Experience in using IT-based patient documentation systems.
* Understanding of health education/promotion.
* Understanding of Confidentiality and the Data Protection Act.
* Understanding of clinical governance and its significance and impact upon practice.
* Ability to participate in audit and standard setting.
* Awareness of equality and valuing diversity principles.
* Evidence of experience/competency in the form of a professional portfolio.
* Experience of working in Primary and Community Care.
* Experience of clinical supervision.
Qualifications
* Full registration with the Health and Care Professions Council (HCPC).
* Minimum of 5 years post-qualifying experience.
* Higher Education Qualification (FHEQ) Level 7.
* Educated to degree level in Paramedic Science.
* Evidence of working at an advanced/specialist paramedic level.
* Evidence of relevant and up-to-date CPD.
* Masters degree in Advanced Clinical Practice or equivalent.
* Independent Non-Medical Prescribing Qualification.
* Member of the College of Paramedics.
* Completed HEE Roadmap.
