Are you an experienced Advanced Clinical Practitioner(ACP) looking for a unique challenge? We have an exciting new role working as part of an integrated team supporting older people with frailty in their own homes and within a community hospital setting.
As part of the Bridport neighbourhood complex frailty team, you will be joining a multi-disciplinary service to provide rapid response and admission avoidance care to our patients. You will be based in Bridport Community Hospital and will work as part of the community and inpatient teams to provide seamless service delivery to achieve our overall objectives.
You will join an excellent senior clinical team supported by a geriatrician and the Bridport Hub. The Bridport Hub offers a truly integrated service with partner agencies including Dorset County Hospital, Dorset County Council, Ageing Well, Older People’s Community Mental Health, Social Prescribing, primary care services, Weldmar Hospicecare, rehabilitation services and community nursing teams.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
To provide highly specialist high-quality person-centred care which always considers people’s safety, privacy and dignity using advanced clinical knowledge that is evidence based.
As part of a multi-professional team, the post holder will practice autonomously, whilst being accountable and self-directed in line with the relevant code of professional conduct.
To plan and manage complete episodes of care; undertake independent comprehensive assessment and management of service users to incorporate pharmacological considerations; making complex clinical decisions regarding service user management and clinical outcomes.
To provide expert clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision making skills.
To lead as a driver for change, to monitor and improve standards through supervision, evidence based practice, clinical audit, research and education. To promote and demonstrate best practice by integrating evidence into practice
To work strategically, contributing to the planning and development of integrated services.
The post holder will be accountable for the direct delivery of autonomous service user care.
