Helping them to achieve as much independence as they can in their own homes. To support this we are looking for a Team Leader for our Home Assessment Team (East Kilbride). You will work a multi-disciplinary team that includes Occupational Therapists, Physiotherapists, Community Nurses and Care at Home professionals with the aim of supporting, motivating and inspiring people to re-learn daily skills and enhance their quality of life. This may be required following a period in hospital or to prevent admission to hospital. As part of the Home Assessment management teams, the Team Leader will play a key leadership and management role, responsible for the management and supervision of a multi-disciplinary team and the management and oversight of the assessment function as well as the management, delivery and development of services.
