This post is an exciting opportunity to support the Community First transformation and partnership between Derbyshire Community Health Services NHS Foundation Trust and Derby City Council.
The post is a job share with an existing manager and will be responsible for the Home First Discharge to Assess pathway 1 and 2a services in Derby City which are provided either in people's own home or as a short stay in Perth House. The services deliver reablement led assessments of long term health and social care needs either following discharge from hospital or in response to preventing an unnecessary admission.
The service has recently transferred from Derby City Council to Derbyshire Community Health Services NHS Foundation Trust (DCHS) and the two organisations are continuing to work in partnership to develop an integrated service with the associated DCHS teams that support Discharge to Assess and Urgent Community Response (admission avoidance). As a member of the Derby City Management Team this role will play a key part in the transformation work, maximising the opportunities around integration and ensuring the delivery of efficient, high quality service., The post holder will be responsible for the professional and operational leadership and improvement of services/sites listed in the addendum attached to this job description, they currently relate specifically to Pathway 1 and Pathway 2 Discharge to Assess services.
Due to the transformation work currently progressing in Derby the management responsibilities may change but will remain connected to the core functions of Discharge to Asses and Urgent Community Response.
The post holder will be responsible for the effectiveness of the sites and services, ensuring the delivery of high quality, patient centred, integrated, cost effective care across the service, having leadership responsibility for the teams.
To be responsible for the provision, co-ordination and development of the services and sites, interpreting relevant SOPs, policies and pathways and working with the General Manager to promote and foster partnership working across the Derbyshire system with commissioners, Primary Care Networks and other stakeholders.
The post-holder will provide operational and professional leadership (within a multi-disciplinary team) to the relevant direct reports and front line teams and be responsible for the development and implementation of quality and performance monitoring arrangements.Overseeing complex operational issues including workforce planning, change management andworkforce engagement.
