Job summary An exciting opportunity has arisen to join a team of pathway and system integration leaders as part of the ICB in Leeds. The teams purpose is to create and draw on a matrix of system experts to help integrate services, pathways, and resources around the needs of populations, improving health outcomes and reducing duplication. The main focus of the role will be people with mental health needs, people with a learning disability and those with neurodivergence. The role will be part of a wider team which also covers childrens services. You will collaborate on programmes across a number of key focus areas both within and outside of this portfolio, as determined by system needs and prioritisation. The post holder will manage a portfolio of services and pathways delegated by the Programme Directors in line with agreed priorities. They will lead and participate in matrix teams across the Leeds Health and Care Partnership to enable the improvement of services and outcomes for agreed populations. The post holder will develop and deliver programmes in support of the Partnership priorities. The post holder will work effectively with colleagues across Leeds and West Yorkshire to ensure developments are well aligned.
