Please note that this role is open to Band 5 RMN's with relevant physical health experience, as well as established Band 6 applicants.
The successful candidate will preferably hold a full UK driving licence. This role requires the successful candidate to carry multiple pieces of equipment and transport blood products and other specimens.
To meet the needs of the service, the successful candidate will cover Monday to Friday, 9am to 5pm. This ensures that staff are available within clinic hours and match the working hours of the CMHT should advice or support be required.
This is an exciting position focusing on the physical wellbeing of our service users with mental health needs. The post holder will lead the physical health care agenda within the Trafford North area, under the supervision of the CMHT Manager and supported by other senior practitioners on the team.
The post holder will ensure the delivery of all physical health care CQUIN requirements and trust key performance indicators. They will act as a specialist resource and advisor to care coordinators within the community teams, working across community settings, including clinic bases and home visits.
The post holder will work, for the majority of the time, as an autonomous clinical practitioner for patient groups across a range of clinical settings. They will be expected to forge strong links with the Care Coordinators, Physical Health Support staff, and Primary Care services, particularly GP practices.

An opportunity has arisen to work in one of our busy, culturally diverse community mental health teams. The CMHTs are multidisciplinary teams, comprising nurses, occupational therapists, psychiatrists, psychologists, social workers, employment specialists, and peer support workers.
