The team works closely with colleagues throughout Oxford Health, the acute trust, and adult social care. The team has also built collaborative links with local and national charities to enhance opportunities for people living with neurological conditions. The service strives to empower service users and their carers to make informed choices about their self-management and to participate in social and leisure activities. Comprehensive assessments are completed in various settings including home visits, clinics, and by digital means.
The post holder will sensitively develop personalised treatment and management plans for those living with long-term neurological conditions. Due regard will be given for the service user's physical and psychological needs, along with the environment in which the care will be delivered. An ability to work autonomously and as a lone worker is vital for this role. The successful candidate will be able to demonstrate exceptional organisational and communication skills, alongside an ability to work flexibly.
The core service currently runs Monday to Friday; however, some very occasional evening working may be required to meet the needs of the service users.
